AZ: LB RMQ Pass-thru to HAProxy disconnect - 11H1 v1

Description
Disconnects the instance from the servers running HAProxy. This script is typically run as either an operational or decommission script to disconnect an application server from the running frontend or dedicated load balancer servers. Once the server has been added to the load balancing pool, HAProxy will start to forward traffic to it. The instance will use the LB_HOSTNAME input to find the existing load balancers. It will then remove itself from the load balancing pool using the LB_APPLISTENER_NAME input.
